Utah Phlebotomy Training Grant

For Utah in-person phlebotomy courses only

A limited number of grants are available for eligible Utah residents to complete OQMPS phlebotomy training in Ogden, Murray, or Orem. Classes are held for three consecutive days, either from 10:00 a.m. to 1:00 p.m. or from 6:00 p.m. to 9:00 p.m. Grant recipients must complete the training, actively pursue employment in phlebotomy or a closely related field, and participate in follow-up regarding training and employment outcomes.

To be eligible you must currently live in Utah, be currently unemployed, and have reliable transportation to class each day. Submitting an application does not guarantee selection or funding.

2. Mandatory eligibility

You must answer Yes to all four to meet the grant requirements.

A. Do you currently reside in Utah? *
B. Are you currently unemployed and not performing paid work for an employer or through part-time, temporary, contract, gig, or self-employment? *
C. Do you have reliable transportation to and from the training location every scheduled class day? *
D. If selected, are you willing to provide reasonable documentation confirming Utah residency and current unemployment if required? *

3. Training location and attendance readiness

Classes meet for three consecutive days, either from 10:00 a.m. to 1:00 p.m. or from 6:00 p.m. to 9:00 p.m.

4. Interest in phlebotomy

Have you previously completed or withdrawn from another healthcare training program?
Phlebotomy training involves needles, blood, close patient contact, practice with classmates, and performing blood draws. Are you prepared to participate fully in these required activities? *

If selected, do you intend to seek employment in phlebotomy or a closely related field after completing the program? *
