Student Waiver and Release of Liability

I, the undersigned, understand and agree to the following terms as a participant in the Oquirrh Mountain Phlebotomy School LLC Online Phlebotomy Training Course:

Assumption of Risk

• I acknowledge that phlebotomy training involves the use of needles, blood collection devices, and other equipment that carry inherent risks, including but not limited to needle-stick injuries, bleeding, bruising, infection, allergic reactions, or other complications.
• I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that may arise from my participation in this course.

Responsibility for Use of Training Kit

• I understand that a phlebotomy training kit will be mailed to me for practice purposes.
• I accept full responsibility for the proper use, storage, and disposal of all equipment, including needles and sharps.
• I agree that Oquirrh Mountain Phlebotomy School LLC is not responsible for any injuries or damages resulting from my handling, misuse, or disposal of the training kit.

Responsibility for Practice Participants

• I acknowledge that it is my sole responsibility to arrange for volunteers (“practice participants”) for my required, video-documented blood draws.
• I affirm that I will obtain informed consent from any person who allows me to practice phlebotomy on them.
• I release Oquirrh Mountain Phlebotomy School LLC, its owners, instructors, and affiliates from any liability for injuries, complications, or claims made by practice participants.

Medical Disclaimer

• This course is provided for educational purposes only.
• I understand that Oquirrh Mountain Phlebotomy School LLC does not provide medical advice, treatment, or patient care as part of this training.

Release of Liability

• In consideration of being allowed to participate in this course, I hereby release, discharge, and hold harmless Oquirrh Mountain Phlebotomy School LLC, its owners, instructors, employees, affiliates, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to my participation in the training course, including injuries to myself or others.

Acknowledgment of Understanding

• I have carefully read this waiver and fully understand its contents.
• I voluntarily signed this document as a condition of enrollment in the Online Phlebotomy Training Course.
• I acknowledge that I'm 18 years of age or older.

To successfully complete the phlebotomy program, every student will be required to perform the tasks and functions in which they are learning.

Oquirrh Mountain Phlebotomy School is a post secondary education facility. We do require that all students who enroll in the phlebotomy course possess a high school diploma, General Education Development (GED) Certificate or be in high school to take the course. Each student is required to sign this waiver release of liability as part of the registration process on this website.

We do not acknowledge credit to students for other programs/courses taken.

We do not guarantee jobs or positions as part of taking this course.

We do not offer Job placement. We do not guarantee hourly wage levels for this type of training.

Refund Policy:
Because this is a digital course, all tuition becomes non-refundable once access to the online course portal or course materials has been granted.
